A Disability Benefits Questionnaire (DBQ) is the form the VA uses to notate the diagnoses that a veteran has. The form is made up of several fillable sections that the VA examiner fills out during the course of a compensation and pension exam. It can have a significant impact on the disability percentage the VA rates a veteran depending on how it is filled out.

Upon agreeing to a Disability Benefit Questionnaire (DBQ) we will send you a contract for services to sign along with a secure online link to pay $200 for medical records review which is a one-time fee (required prior to obtaining a DBQ).
Once we receive the signed contract and payment we will send you a secure link to our secure file sharing portal for you to electronically upload your medical records, service record and any buddy or lay statements you would like to add. We will have one of our medical experts complete a medical records review to determine if we can assist in completing a DBQ. If it is determined that we can help, we will get a hold of you and send you a secure online link to pay $150 for each DBQ that you require.
Once we have received your payment one of our medical experts will fill out the DBQ or DBQ’s with information that you can provide to the compensation and pension examiner. We will then send you an electronic copy of the DBQ or DBQ’s to take with you to your compensation and pension exam.
